Bank charges for individual accounts - are you being overcharged?

My cousin arrived in India for his MBA degree and opened a bank checking account in USA with a bank to receive an ATM debit card. He could receive transfers directly into checking account and was able to withdraw funds instantly from any ATM in India or worlwide.   Paying a monthly fees $5, and bank charges of $5 per withdrawal transaction. This practically restricts withdrawal of money once or twice in a month for him.  He was and is very happy with the service. But  paying $5 monthly fees should include one free withdrawal every month.

After the first semester, he had a 2 month break. He did not use the first debit card for 4 months for bank transfers and withdrawals. On the next transfer after four months the bank deducted $20 as bank charges at $5/month for 4 months. May be they have a policy, but I have yet to confirm if a charge a fees of $5/month for the months no transaction is made within the allowed bank practice.
